Clearance Information Sheet

NOTE: DO NOT REGISTER AS A VOLUNTEER if you are being paid in your position.

1. ACT 114 – FBI Clearance – Register with Identogo through their website: https://www.identogo.com.

Use service code 1KG6XN. Take your receipt and a valid photo ID to an Identogo office (office locations are listed on their website or call them toll free at 844-321-2101).

Once your prints have been taken, please provide a copy of your receipt containing your UE-ID number to: Mrs. Cindy Donovan, Avonworth School District, 258 Josephs Lane, Pittsburgh., PA 15237.

2. ACT 34 – Pennsylvania Criminal History Check Website: https://epatch.pa.gov

Go to “New Record” and follow the instructions through to the Certification Page. Once the Search Results Table appears, click on the Control Number. (Write down the Control Number for future reference.) The Record Check Details page is opened. Click on the words “Certification Page” in the middle of the screen to access your official Clearance. It doesn’t look like a button to click but it is if you hover over it. The Record Check Details page is only a receipt and not acceptable as a ‘clearance’. Print the Certification Page. PATCH Helpdesk 1-888-QUERY-PA (1-888-783-7972).

Please provide a copy of your PA Criminal Clearance to Mrs. Cindy Donovan, Avonworth School District, 258 Josephs Lane, Pittsburgh., PA 15237.

3. ACT 151 – Pennsylvania Child Abuse Clearance

To apply for your Child Abuse clearance online go to www.compass.state.pa.us/cwis/public/home See “Welcome to the Child Welfare Portal” and click on “Create Individual Account.” NOTE: On the “Create Keystone ID: Profile Information page, you’ll need to create your own Keystone ID. They will email you a temporary password. You will need to go back into the site and enter that temporary password and follow the instructions. Call the Compass help line at 1-877-371-5422 with questions or for assistance.

Please provide a copy of your PA Child Abuse Clearance to Mrs. Cindy Donovan, Avonworth School District, 258 Josephs Lane, Pittsburgh., PA 15237.

4. Act 126 of 2014, Mandated Child Abuse Reporter, Pennsylvania State Law

In accordance with Act 31 of 2014, as a school employee that interacts with students, you are required by the state to be a mandated child abuse reporter. Being a mandated reported requires you to report any form of child abuse that you have a reasonable cause to suspect. Free Online Act 126 Training

You may complete the training online at this link: http://www.reportabusepa.pitt.edu This is offered at no cost through the University of Pittsburgh. Upon completing this training, be sure to print your certificate and provide a copy to Mrs. Cindy Donovan, Avonworth School District, 258 Josephs Lane, Pgh., PA 15237.

5. Act 168 Sexual Misconduct/Abuse Disclosure Release

Complete side 1 of the Act 168 form and sign on page 2. If you have not worked with children, check the box near the top of side 1 indicating ‘No applicable employment.”

Send completed form to Mrs. Cindy Donovan, Avonworth School District, 258 Josephs Lane, Pittsburgh, PA 15237. She will send the form to your previous employers unless you checked the “No applicable employment” box.

6. Act 24 – Arrest/Conviction Report and Certification Form #6004

This form should be completed if your clearances are over a year old and returned to Mrs. Cindy Donovan, Avonworth School District, 258 Josephs Lane, Pittsburgh., PA 15237.
